Notice on Applying for Health Declaration Form Online

According to a November 2 notice put up on the embassy website, effective from November 6, 2020, all Chinese and foreign passengers who are flying to China from Dominica will be required to take nucleic acid and IgM anti-body tests and then apply for a green health code with the "HS" mark or a certified health declaration form before boarding. However, for the convenience of foreign travelers to China, foreign nationals are going to apply online for an Electronic Health Declaration Form displayed in QR code with the "HDC" mark, instead of a paper Health Declaration Form. The application procedures are as follows:

Starting from 20 November,2020, all foreign passengers (including the Fast Lane passengers) who are flying from Dominica to China will be required to present the Electronic Health Declaration Form for boarding. Foreign passengers can apply for the Electronic Health Declaration Form by logging on the website of http://hrhk.cs.mfa.gov.cn/H5/ via computer or smart phone or you can scan the following QR code. Foreign passengers can fill in the information, declare the health status and upload the testing certificates online, which will then be examined and verified by the Chinese Embassy. Upon approval, they will receive the Electronic Health Declaration Form, which will be displayed in QR code with the "HDC" mark. Foreign passengers must present the electronic or printed code, and follow the procedures for inspection by relevant airlines during boarding.

As from November 20, 2020, it is no longer viable to apply for a Health Declaration Form by means of emailing the embassy.

It is important to note that whether or not they are entering the customs of the transit country (last country of origin for direct flights to China), passengers are required to take nucleic acid and IgM anti-body tests in the transit country, and then, as aforementioned, apply online for an Electronic Health Declaration Form displayed in QR code with the "HDC" mark, which should be presented to the airlines check-in staff before boarding.
